Job Description

Hebrew SeniorLife provides enhanced housing with services to seniors across four affordable housing locations and partner sites. Our proactive approach involves resident services staff actively engaging with residents to address their health and wellness needs, offering personalized case management and support to foster independence and safety.

Role Overview: Wellness Coordinator (WC) 💼

The Wellness Coordinator leads the R3 program by conducting wellness assessments, delivering health education, connecting residents with necessary services, and coordinating with healthcare providers. The role requires excellent communication and collaboration skills to work effectively with emergency responders, payers, and local providers, ensuring residents can live safely and independently for as long as possible. This position is based on-site at an affordable housing community in Chelsea and requires fluency in Spanish due to the resident population.

Position Responsibilities

  • Conduct wellness assessments and develop personalized wellness plans covering physical, social, cognitive, spiritual, and mental health domains
  • Coordinate with primary care, mental health providers, and hospitals
  • Partner with housing staff to tailor resident programming
  • Educate staff on recognizing and communicating resident health changes
  • Assist residents with completing health records, end-of-life planning, and emergency contacts
  • Follow up with at-risk residents to ensure adherence to wellness activities
  • Build relationships with payers and ensure effective communication systems with providers
  • Promote self-care among residents through coaching and personalized services
  • Make referrals to local service providers such as Visiting Nurses Associations, Rehabilitation Services, and Memory Programs
  • Lead or participate in care planning meetings
  • Assess needs of residents with dementia and facilitate appropriate referrals
  • Collaborate with the Nurse Care Manager on evidence-based programs
  • Ensure safe discharges with coordinated services alongside hospitals and home care agencies
  • Support residents through care transitions and family meetings
  • Advocate for residents and serve as a liaison with families
  • Track key metrics and prepare outcome reports

Qualifications

  • Master’s degree in social work or a related human services field with 1-2 years of case management experience in aging services (preferred); Bachelor’s degree with 3-5 years of experience required
  • Passion for working with seniors and compassionate approach
  • Excellent organizational, interpersonal, and multitasking skills
  • Professional, collaborative, proactive, and results-driven mindset
  • Positive attitude with strong communication and judgment skills
  • Motivated to learn and adaptable to change
  • Competency in documenting clearly in AASC Online software
  • Proficiency with Windows, Word, PowerPoint, and Excel
  • Willingness to travel within the Boston metro area
  • Certification in Dementia Care (preferred)
  • Fluency in Spanish is required 🗣️

Salary Range: $53,971.45 - $80,957.18
