Job Summary
The Social Worker I will work autonomously, developing one-on-one relationships with residents to deliver comprehensive social services. The role aims to support residents in maintaining safe, independent lifestyles within the community.
Professional Development & Support
WSFSSH encourages MSW-level social workers to obtain licensure. We offer:
- Study groups and financial support for exam preparation
- Payment for up to 2 licensing exams and registration fees
- Coverage of CEU credits after LMSW certification
- Weekly clinical supervision by an LCSW
- Pathway from LMSW to LCSW, with generous support for licensing fees and CEUs
Key Responsibilities
- Resident Support: Assist with medical, mental health, entitlement services, home care, family/social relationships, budgeting, and money management.
- Crisis & Clinical Services: Provide crisis intervention, client appointment escorts, and individual counseling.
- Community Engagement: Facilitate resident participation in social activities through referrals and outreach.
- Follow-up & Coordination: Monitor residents during hospitalizations or rehab placements, and manage discharge planning processes.
- Group Services: Lead recreational, educational, and clinical group sessions.
- Intake & Documentation: Participate in resident intake, share progress updates, and maintain detailed case records.
- Compliance & Safety: Complete incident reports, implement emergency procedures, and attend meetings.
- Other Duties: Perform additional tasks as assigned, ensuring excellent resident service.
Required Knowledge, Skills & Abilities
- Strong written and verbal communication skills
- Excellent organizational, time management, and interpersonal abilities
- Genuine interest in supporting the aging population
Qualifications
- MSW or MA in human services with at least 2 years of social work experience
- Bilingual in English and Spanish (fluency required)
- Experience with older adults, mental health, or substance use is preferred
Physical Requirements
This role may include physical demands such as prolonged sitting or standing and extensive computer use. Ability to stand for long periods is required.
