Job Summary ✨
The Social Worker I operates autonomously, developing one-on-one relationships with residents to deliver a comprehensive range of social services. Our goal is to help residents establish and sustain safe, independent lifestyles within the community.
We strongly support M.S.W. level social workers in obtaining their licensing, offering study groups, job-paid courses, and assistance with exam and licensing fees. Once licensed, WSFSSH continues to support professional development with CEUs, supervision, and pathways toward advanced licensure such as the LCSW.
Key Responsibilities 📝
- Assist residents with:
- Accessing and maintaining medical, mental health, entitlements, and home care services
- Establishing and maintaining family and social relationships
- Budgeting and rent payment management
- Money management
- Provide crisis intervention, client escorts, and individual counseling as needed 🚑🛡️
- Coordinate socialization activities through in-house and community referrals 🎉
- Follow up during hospitalizations or rehab stays, ensuring continuity of care
- Oversee discharge planning including forms and applications to other facilities
- Lead group services (recreational, educational, clinical) for residents 🧑🤝🧑
- Participate in the intake of prospective residents
- Share resident progress and needs with team members, following confidentiality policies 🔒
- Complete incident reports and maintain thorough case documentation 📝
- Implement emergency procedures, attend meetings, and perform other duties as assigned
Qualifications & Skills ✅
- Excellent written and verbal communication skills 🗣️✍️
- Organizational skills, time management, and interpersonal abilities
- Interest in supporting aging populations ❤️
Educational & Experience Requirements 🎓
- MSW or MA in human services with at least 2 years of social work experience
- Fluency in English (written and oral); bilingual English/Spanish required 🌎
- Experience working with older adults or individuals with mental health/substance use issues preferred
Physical Requirements 🧍♂️🖥️
- Sitting or standing for extended periods
- Heavy computer use
- Ability to stand for long durations
