📝 Job Description

Basic Function: The Social Worker will develop services using the Harm Reduction model of care, emphasizing tenants' choice in service delivery. While sobriety and engagement are encouraged, they are not mandatory. The role involves trauma-sensitive case management, including assessments, treatment planning, linkages, and referrals to various services.

💼 Major Duties

• Conduct assessments: Complete comprehensive bio-psycho-social evaluations covering areas such as substance use, health, mental health, relationship abuse, social services, family, schooling, and employment.
• Coordinate services: Collaborate with on-site Health Coordinator, Substance Use Specialist, Vocational/Educational Specialist, and external providers.
• Liaison activities: With appropriate written consent, coordinate with tenants' treatment providers to ensure seamless service delivery.
• Develop service plans: Create individualized plans with clear goals and outcomes aimed at supporting tenants in maintaining housing stability.
• Provide case management: Assist tenants with daily living skills, crisis intervention, financial management, substance use, mental health, and advocacy for public benefits and entitlements.
• Support housing stability: Help establish household and other services to promote housing retention.
• Therapeutic services: Offer individual and group therapy to aid in overcoming dependencies, adjusting to life, or making necessary changes.
• Additional duties: Perform all related and assigned duties, whether related or unrelated to the core responsibilities.

📅 Schedule & Compensation

Schedule: Monday through Friday, 9:00 AM - 5:00 PM

Salary: $60,000 - $64,000 annually, depending on experience
