🔹 Job Description

Basic Function: The Social Worker will develop services utilizing the Harm Reduction model, emphasizing tenant choice, engagement, and trauma-sensitive case management. The role includes assessing client needs, creating treatment plans, and facilitating linkages and referrals to appropriate services, with sobriety not being a mandatory requirement.

🔹 Major Duties & Responsibilities

• Conduct comprehensive bio-psycho-social assessments covering substance use, health, mental health, relationship abuse, social services, family dynamics, education, and employment.
• Coordinate services with on-site professionals such as the Health Coordinator, Substance Use Specialist, Vocational/Educational Specialist, and external providers.
• Liaise with treatment providers in accordance with written consents to ensure seamless service coordination.
• Develop individualized service plans with clear goals and outcomes to support tenants in maintaining housing stability.
• Provide case management services in collaboration with tenants on daily living skills, crisis intervention, financial management, substance use, mental health, and public benefits advocacy.
• Assist tenants in establishing household routines and other services to promote housing stability.
• Offer individual and group therapy to support tenants in overcoming dependencies, adapting to life changes, or making positive modifications.
• Perform additional duties as required, whether related or unrelated to the primary role.

🔹 Schedule & Compensation

Schedule: Monday to Friday, 9:00 AM - 5:00 PM

Salary: $60,000 - $64,000 annually, depending on experience

Location: Bronx, NY 10456
