Job Description

🔹 Basic Function

The Social Worker will develop services using the Harm Reduction model of care, emphasizing tenant choice in service delivery. While sobriety and tenant engagement are supported, they are not mandatory. The role includes overall trauma-sensitive case management, which involves assessment, treatment planning, linkages, and referrals to appropriate services.

🔹 Major Duties

• Conduct comprehensive bio-psycho-social assessments covering:
  • Substance use
  • Health and mental health
  • Relationship abuse
  • Social services, family, schooling, and employment
• Coordinate services with:
  • On-site Health Coordinator
  • Substance Use Specialist
  • Vocational Educational Specialist
  • External providers
• Liaise with tenants' treatment providers using appropriate written consents to coordinate services effectively.
• Develop individualized service plans with clear goals and outcomes aimed at maintaining housing stability.
• Provide case management services in consultation with tenants, including:
  • Daily living skills
  • Crisis intervention
  • Financial management
  • Substance use and mental health support
  • Public benefits and entitlements advocacy
• Support tenants in establishing household and other services to promote housing stability.
• Conduct individual and group therapy sessions to aid in overcoming dependencies, adjusting to life, or making positive changes.
• Perform all related or assigned duties as needed.

🔹 Schedule & Compensation

Schedule: Monday to Friday, 9:00 AM - 5:00 PM

Rate: $60,000 - $64,000 per year, dependent on experience
