Position Summary
We are looking for a bilingual Russian-speaking and highly motivated Service Coordinator to join our Nursing Home Transition and Diversion (NHTD) Waiver Program team. In this role, you will assist individuals with physical disabilities and seniors in accessing community-based services that promote independent living and prevent unnecessary institutionalization. Your responsibilities include conducting assessments, developing and monitoring service plans, coordinating community resources, and advocating for participants while ensuring compliance with NYSDOH regulations and NHTD standards.
Key Responsibilities 📝
- Serve as the primary point of contact for patients, families, healthcare providers, and community agencies to coordinate services effectively.
- Conduct assessments of participant needs, strengths, goals, and eligibility on an ongoing basis.
- Develop, implement, and monitor individualized Service Plans (ISPs) and Revised Service Plans (RSPs) collaboratively with participants and interdisciplinary teams.
- Coordinate services and referrals such as housing, home care, medical, behavioral health, transportation, and community supports.
- Maintain regular contact with participants via home visits, phone calls, and community meetings.
- Advocate for participant rights, independence, and informed decision-making.
- Monitor health and welfare to ensure services are effective and appropriate.
- Ensure accurate documentation in line with NYSDOH and agency standards.
- Participate in interdisciplinary meetings, trainings, supervision, and quality assurance activities.
- Assist participants transitioning from institutional settings into the community.
- Stay informed about Medicaid Waiver services, community resources, and eligibility criteria.
Qualifications 📋
To qualify as a Service Coordinator, candidates must meet one of the following educational and experience requirements:
- Licensed master’s or clinical social worker or Doctorate/Master of Social Work with a minimum of 1 year of case management experience.
- Master of Gerontology or Licensed Physical Therapist or Registered Nurse with at least 1 year of relevant experience.
- Other NYS licensed professionals such as Teachers of Students with Disabilities, Certified Rehabilitation Counselors, Speech Pathologists, or Occupational Therapists with relevant experience.
- If holding a Bachelor’s degree, candidates must have 3 years of experience.
- If holding an Associate’s degree, candidates require 5 years of experience.
Additional preferred qualifications:
- Experience working with disabilities, seniors, or Medicaid populations
- Knowledge of the NHTD Waiver Program and community services
- Strong organizational, communication, and documentation skills
- Ability to work independently with an effective caseload management
- Proficiency in Microsoft Office and electronic documentation systems
- Valid driver’s license and reliable transportation (preferred)
- Bilingual Russian Speaking is a must
