Job Summary
As a Coordinator at Elite Home Health Care, you will play a vital role in coordinating home care services efficiently for our patients. Your responsibilities include managing caseloads, coordinating care placements, maintaining records, and supporting staffing needs—all to ensure clients receive timely, compassionate care. Your leadership and communication skills will help create a positive environment for patients, families, and team members alike. 🌟🤝
Essential Duties and Responsibilities
- Manage your assigned caseload, ensuring optimal home care services for patients."
- Coordinate service placement between clients and caregivers using HHA eXchange.
- Maintain effective caseload management to deliver timely and appropriate care.
- Participate in departmental training and meetings to stay updated on policies and practices.
- Support the team lead with scheduling coverage and collaborate to resolve staffing issues.
- Communicate openly and constructively with staff regarding schedules, availability, and attendance.
- Accurately update patient and caregiver records in HHA eXchange.
- Verify caregiver attendance daily and prioritize staffing based on patient needs.
- Handle calls, emails, or faxes from contractors and clients, providing updates and discussing patient status with nurse care managers.
- Address caregiver concerns about working conditions or policies, collaborating with management to resolve issues in the best interest of employees.
- Assist with on-call duties as scheduled.
Qualifications and Skills
- Experience in healthcare or related fields is preferred.
- Proficient with Microsoft Office Suite, HHA Exchange, and internet navigation.
- Strong leadership and problem-solving abilities.
- Excellent organizational skills and attention to detail.
- Effective oral and written communication skills for liaising with employees, patients, and families.
- Ability to identify problems, gather data, establish facts, and take appropriate action.
- Able to multitask effectively in a fast-paced environment.
- Flexible schedule to meet department and patient needs.
Physical Demands

Pay Range
The starting pay range is $25.00 – $27.00/hour. Final compensation depends on relevant experience, skills, certifications, and location.
