Job Summary

As a Coordinator at Elite Home Health Care, you will play a crucial role in the efficient management and coordination of home care services. Your responsibilities include managing caseloads, coordinating service placement between clients and caregivers, maintaining records, and supporting staffing needs. Your leadership, communication, and problem-solving skills will ensure high-quality support for our patients and their families.

Essential Duties and Responsibilities:

  • 🔹 Manage an assigned caseload to ensure patients receive optimal home care services
  • 🔹 Strategically coordinate placement of services using HHA eXchange
  • 🔹 Maintain effective caseload management for timely and appropriate patient care
  • 🔹 Participate in departmental training and meetings to stay current on policies and best practices
  • 🔹 Assist team lead with coverage and collaborate on staffing solutions
  • 🔹 Openly and constructively discuss schedule, availability, attendance, and other crucial issues with staff
  • 🔹 Update patient and caregiver records promptly and accurately in HHA eXchange
  • 🔹 Verify caregiver attendance daily; prioritize staffing for no-shows or call-outs based on patient needs
  • 🔹 Handle calls, emails, or faxes from contractors and clients regarding reports or information; coordinate with nurse care managers on patient status
  • 🔹 Address caregiver concerns about working conditions or policies; collaborate with management to resolve issues
  • 🔹 Assist with on-call duties as scheduled

Qualifications and Skills:

  • 🔹 Experience in healthcare or related fields preferred
  • 🔹 Proficiency with Microsoft Office Suite, HHA eXchange, and internet navigation
  • 🔹 Strong leadership and problem-solving abilities
  • 🔹 Excellent organizational skills with keen attention to detail
  • 🔹 Effective oral and written communication skills for interaction with staff, patients, and families
  • 🔹 Ability to identify problems, analyze data, establish facts, and implement solutions
  • 🔹 Capable of multitasking in a fast-paced environment
  • 🔹 Flexible schedule to meet departmental and patient needs

Physical Demands:

  • 🖥️ Remain stationary for long periods at a desk or computer
  • 🚶‍♂️ Occasionally stand, sit, walk, reach with hands and arms, climb stairs, hear, and talk

Compensation:

The hiring range for this position is $25.00 - $27.00 per hour. Final pay will depend on
relevant experience, skills, certifications, and location.
