We are looking for a compassionate, organized, and tech-savvy LPN or RN fluent in Russian and English to support our Nurse-Led Annual Wellness Visit (AWV) Program. 🌟 About the Role

This position focuses on engaging patients, coordinating preventive and chronic care, and assisting healthcare providers in delivering high-quality, value-based services through telehealth and remote platforms. The ideal candidate is adept at navigating multiple EMR systems, telehealth tools, payer portals, and documentation workflows, all while adhering to scope of practice guidelines under provider supervision.

📝 Primary Duties & Responsibilities

• Prepare and conduct Medicare Annual Wellness Visits (AWV) and nurse-led patient assessments under supervision
• Engage patients via phone and telehealth platforms to complete health risk assessments, screenings, and preventive reviews
• Document accurately within multiple EMR/EHR systems in compliance with workflows and policies
• Manage multiple technological systems simultaneously, including EMR/EHR, telehealth software, care management platforms, payer portals, and scheduling tools
• Identify and help close preventive care gaps for patients
• Support care coordination efforts, especially for chronic disease management populations
• Educate patients on preventive services, wellness goals, medication adherence, and follow-up plans
• Escalate clinical concerns appropriately to supervising providers or clinical leadership
• Ensure compliance with HIPAA, CMS guidelines, and organizational documentation standards
• Assist in creating care plans and protocols within nursing scope of practice
• Coordinate scheduling and follow-up appointments as needed
• Collaborate with interdisciplinary teams to enhance patient outcomes and engagement
• Maintain professional communication with patients, caregivers, providers, and staff

🎯 Preferred Qualifications

• Active LPN or RN license in good standing
• Fluent in Russian and English (spoken and written) 🗣️
• Experience with Medicare AWV preferred
• Strong understanding of EMR/EHR systems and telehealth workflows
• Excellent communication and patient engagement skills
• Ability to multitask efficiently in a fast-paced remote or hybrid setting
• Strong organizational and documentation skills

💰 Compensation & Benefits

Pay: $28.00 - $60.00 per hour
