Seeking a dedicated and compassionate Registered Nurse to coordinate care and empower members across Manhattan, Brooklyn, Queens, or Bronx, with a flexible hybrid work schedule.

💼 Position Details

• Compensation Range: $100,000 - $115,000
• Location: Manhattan, Brooklyn, Queens, or Bronx territories; 4 days remote, 1 day in the field
• Travel: Up to 25% travel to homes, offices, or public settings as needed
• Schedule: Monday – Friday, Business Hours
• Type: Permanent, Full-Time

🚑 Position Summary

The Nurse Care Manager plays a vital role in providing comprehensive care coordination, including in-home assessments, planning, advocacy, and authorization of covered services. The goal is to meet members' health needs while promoting quality, cost-effective outcomes.

📝 Key Duties & Responsibilities

• Assess and monitor members’ needs, ensuring consistent care across the healthcare continuum
• Authorize covered services and coordinate care regardless of payer
• Collaborate with members, families, caregivers, primary care providers, and interdisciplinary teams
• Assist members in maintaining the highest level of independence possible
• Conduct home visits to evaluate living situations, cultural influences, and cognitive and functional needs
• Develop and implement Person-Centered Service Plans in collaboration with the physician and IDT
• Ensure safe and appropriate discharge planning
• Identify, investigate, and document grievances and complex care needs
• Respond promptly to member requests and complete adverse determinations as required
• Evaluate members needing Care Management Review and develop tailored care plans
• Maintain thorough documentation of care activities, including telephonic and in-person recertifications
• Develop efficient care plans, utilizing network providers to meet members' needs effectively
• Perform additional job-related duties as needed

🎓 Requirements

👩‍⚕️ Education & Experience

• Associate’s degree in Nursing from an accredited program (required)
• Bachelor’s degree in Nursing (preferred)
• Minimum 3 years of RN experience (required)
• Geriatrics or managed long-term care experience (preferred)
• Multilingual abilities may be required based on operational needs

📝 Licenses & Certifications

• Current, unrestricted RN license in New York State (required)
• Case Management Certification (preferred)

• Intermediate proficiency with Microsoft Office Suite (Outlook, Word, Excel)
• Effective communication with diverse populations
• Ability to multitask and work independently within a team
• Knowledge of healthcare laws, regulations, and policies

• Advocacy for members at all care levels
• Cultural sensitivity and awareness of demographic diversity
• Interpretation of current research & community resources
• Critical thinking, problem-solving, and organizational skills
• Bilingual in speaking and writing (highly preferred)
