📝 Compensation & Location
- Salary Range: $100,000 - $115,000
- Locations Available: Manhattan, Brooklyn, Queens, or Bronx
- Work Schedule: 4 days remote, 1 day field-based per week
- Travel: Up to 25% regular travel to homes, offices, or public settings
📋 Position Summary
The RN Care Manager is responsible for delivering comprehensive care coordination, including in-home assessments, planning, advocacy, and authorization of services. The goal is to meet members' health needs while promoting quality, cost-effective outcomes.
🕒 Schedule & Status
- Shift: Monday through Friday, Business Hours
- Type: Permanent, Full-Time
🎯 Core Responsibilities
- Ensure seamless care across all healthcare settings by assessing and monitoring members’ needs.
- Authorize services and coordinate care regardless of payer.
- Collaborate with members, families, caregivers, primary care providers, and interdisciplinary teams.
- Support members in maintaining the highest level of independence possible.
- Perform home visits to evaluate living conditions, cultural factors, and cognitive needs.
- Work with physicians and the interdisciplinary team to develop personalized care plans.
- Manage members' discharge planning to ensure safe transitions.
- Investigate and document grievances according to plan policies.
- Participate in Intensive Care Management meetings for complex cases.
- Respond promptly to member requests and complete adverse determination documentation.
- Identify and evaluate members requiring Care Management Review (CMR).
- Document all care activities in accordance with company policies.
- Develop efficient, needs-based care plans utilizing network providers and appropriate service levels.
- Perform additional duties as needed to support team and organizational goals.
🎓 Requirements
Education & Experience
- Associates degree in Nursing from an accredited program (required)
- Bachelor's degree in Nursing (preferred)
- Minimum of 3 years’ experience as a registered nurse (required)
- Geriatrics or managed long-term care experience (preferred)
- Ability to communicate in multiple languages may be required based on operational needs
Licensure & Certifications
- Current, unrestricted RN license in New York (required)
- Case Management Certification (preferred)
Skills & Competencies
- Proficiency in Microsoft Office Suite (Outlook, Word, Excel)
- Excellent communication with diverse populations
- Strong multi-tasking and independent working skills
- Knowledge of healthcare laws, regulations, and policies
- Adherence to professional ethics and CMSA standards
- Advocacy for members and cultural sensitivity
- Ability to interpret research and utilize community resources
- Critical thinking, decision-making, and organizational skills
- Bilingual skills in speaking and writing are highly preferred
