Position: Registered Nurse - Bilingual Spanish Required

Role Overview

The primary goal of a Registered Nurse at Oak Street Health is to foster strong patient relationships by coordinating care and providing a seamless, personalized health experience. You will utilize an integrated approach, applying standards, guidelines, and pathways to achieve optimal patient outcomes. Through clinical assessment, intervention, and education, you will ensure that our patients receive timely, competent nursing care. Your efforts will drive quality improvement and foster an engaging environment where team communication and delegation support the delivery of excellent care.

Note: Registered Nurses report to the Practice Manager or Nurse Supervisor.

Core Responsibilities 📝

  • Provide proficient nursing care with safety, consistency, and competency.
  • Respond promptly to telephonic requests to meet patient needs and exceed expectations.
  • Perform clinical triage and symptom management, whether patients are present physically or virtually.
  • Utilize standardized protocols for medication management, refills, and prior authorizations.
  • Review and update medications thoroughly and accurately.
  • Educate and care for patients regarding chronic conditions, via in-person, phone, or group sessions.
  • Collaborate and monitor the care plan implementation for patients on multiple provider panels.
  • Build relationships with patients and caregivers to support preventive care and reduce ER/hospital visits where appropriate.
  • Create a welcoming environment tailored to the needs of our diverse communities, families, and teams.
  • Delegate activities to team members to support patient care.
  • Participate in care team meetings to discuss patient progress and clinic operations.
  • Deliver exceptional patient experiences through attentive service and respectful care.
  • Perform point-of-care testing, procedures, and specimen collection (including phlebotomy) as needed.
  • Perform other duties as assigned.

What We're Looking For 🔍

Required Qualifications

  • Active RN licensure in good standing in the relevant state
  • BLS Certification
  • Experience with Electronic Medical Records (EMR)
  • Ability to maintain patient confidentiality and handle sensitive information
  • Authorized to work in the US
  • Strong communication and assessment skills, capable of evaluating patients remotely

Preferred Qualifications 🌟

  • Ability to collaborate effectively within interdisciplinary care teams
  • Proficient computer skills, capable of interpreting data from various systems
  • Excellent problem-solving and prioritization skills
  • At least 2 years of healthcare experience as an RN

Additional Preferred Skills 👍

  • Experience in a clinical setting
  • Ability to work independently
  • Fluency in Spanish, Polish, Russian, or other languages relevant to our communities

Expected Weekly Hours: 40

Employment Type: Full-time

Compensation 💰

The typical pay range for this role is:

$72,627.00 - $155,538.00

This range reflects base salary or hourly rates depending on experience, education, and location. Incentives such as bonuses or commissions may also apply.
