Role Description
The Center Nurse at Oak Street Health is focused on building strong relationships with patients by coordinating their care and providing a seamless healthcare experience. Using an integrated approach, you'll utilize standards, guidelines, and pathways to achieve optimal patient outcomes through clinical assessment, intervention, and education. Your role is vital in driving quality care, leveraging data to improve outcomes, and creating a welcoming environment where team collaboration empowers the delivery of exceptional patient care. 🚑

Our Center Nurses report to the Practice Manager or Nurse Supervisor (where applicable).
Core Responsibilities
- Provide competent nursing care by demonstrating proficiency in clinical skills, safety, and consistency.
- Respond promptly to telephonic requests, exceeding patient expectations with dependable service.
- Offer triage and symptom management for both in-person and remote patients.
- Utilize standardized protocols for medication management, prescription refills, and prior authorizations.
- Review and update medication lists accurately.
- Deliver patient education and direct care, particularly around chronic conditions, via in-person, phone, or group sessions.
- Collaborate and monitor care plans for patients managed by multiple providers.
- Build relationships with patients and caregivers to support preventative care and ED/hospital diversion when appropriate.
- Create a welcoming environment to meet the diverse needs of patients, families, and communities.
- Delegate activities to clinical team members to support patient care.
- Participate in team meetings to discuss patient care and clinic operations.
- Ensure an exceptional patient experience through service, responsiveness, and respectful care.
- Perform point-of-care testing, procedures, and specimen collection (including phlebotomy).
- Perform other duties as assigned.
What we're looking for
Required Qualifications
- Active Registered Nurse (RN) licensure in good standing with the applicable state.
- BLS Certification.
- Experience with Electronic Medical Records (EMR).
- Ability to maintain patient confidentiality and process information securely.
- U.S. work authorization.
- Strong assessment and communication skills for patient interaction without face-to-face contact.
Strongly Preferred Qualifications
- Ability to collaborate effectively within an interdisciplinary care team.
- Excellent computer skills for data interpretation.
- Strong problem-solving and prioritization capabilities.
- At least 2+ years of healthcare experience as an RN.
Preferred Qualifications
- Previous clinic setting experience.
- Ability to work independently.
- Fluency in languages spoken by the community, such as Spanish, Polish, or Russian (optional based on demographic needs).

- Anticipated Weekly Hours: 40
- Time Type: Full time
- Pay Range: $72,627.00 – $155,538.00 annually, depending on experience and location.
This position qualifies for additional bonuses, commissions, or incentives, based on performance.
