About Oak Street Health
At Oak Street Health, our goal is to provide equitable and effective value-based healthcare tailored to local Medicare populations through our network of neighborhood primary care centers. Our primary care physicians work within focused teams at single centers, allowing for high-quality care focused on outcomes rather than volume. Our unique care model emphasizes preventive medicine, reducing hospitalizations, and delivering exceptional patient experiences.
We're seeking physicians who are intrinsically motivated to care for older adults with excellence. New graduates are encouraged to apply, and highly qualified candidates might also be considered for a Center Medical Director role.
Role & Responsibilities
- Assess and diagnose patients at our community clinics 🩺
- Oversee, direct, and administer primary care services
- Prescribe and manage pharmaceutical treatments and medications 💊
- Maintain Patient Electronic Medical Records via Canopy and Greenway systems
- Collaborate with regional and central leadership to meet quality health goals 🤝
- Work with Practice Managers to lead the care team effectively
- Perform other duties as assigned
Required Qualifications
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.)
- Graduate training in Internal Medicine or Family Medicine
- Board Certification or eligibility in respective specialty 🎓
- Active, unrestricted State Medical License
- Active DEA license
- US work authorization
Preferred Qualifications
- Fellowship training in Geriatrics
- Experience practicing medicine with Geriatric populations
- Experience in a Value-Based Healthcare Model
- History of working collaboratively to improve health outcomes
- Experience in outpatient primary care settings
- Bilingual proficiency where applicable 🌐
Details & Compensation
Weekly Hours: 40 hours
Employment Type: Full-time
Pay Range: $174,070.00 – $374,920.00 annually
This range reflects the base salary or hourly rate; actual compensation depends on experience, education, location, and other factors.
