Work Responsibilities & Schedule

• Focus: Outpatient care only
• Schedule: Monday to Friday, no nights or weekends
• Patient Panel: Small, 12-15 patients per day for consistency and quality of care
• Care Model: Value-based care with outcomes-driven bonuses

Job Focus

Provide personalized primary care for older adults on Medicare, aiming to keep patients healthy and living life to the fullest.
