What You Will Do 📝

• Practice independently within the community as part of an interdisciplinary care team
• Conduct comprehensive patient assessments to determine needs and develop treatment plans
• Deliver personalized rehabilitation therapies in patients’ homes or care facilities
• Continuously evaluate patient progress and adjust plans of care accordingly

Qualifications ✅

• Minimum of one year of physical therapy experience or successful completion of a probationary period as a VNS Health Rehab Intern
• Licensure to practice as a Physical Therapist in New York State
• Valid driver’s license required for locations outside Manhattan

Pay Range per Visit 💰

For the per diem, per visit position, the standard pay varies by location:

• All areas except the Bronx and certain Brooklyn neighborhoods (Downtown Brooklyn, Williamsburg, Greenpoint): $70–$75 per visit
• Bronx: Weekday visits – $95 | Weekend visits – $100
• Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint): Weekday visits – $95 | Weekend visits – $100

Annual Salary 💵

Full-time positions offer an annual salary range of USD $85,000.00 - USD $127,500.00.
