What You Will Do

• Practice independently within the community as part of an interdisciplinary care team 🤝
• Perform comprehensive patient assessments, identify needs, and develop tailored treatment plans 🎯
• Deliver personalized rehabilitation therapy sessions at patients' homes or care facilities 🏡
• Continuously evaluate patient progress and adjust plans of care accordingly 🔄

Qualifications

• Minimum of one-year physical therapy experience or successful completion of a probationary period as a VNS Health Rehab Intern ✔️
• Valid licensure to practice as a Physical Therapist in New York State 📜
• Valid driver’s license required for locations outside Manhattan 🚗

Pay Range per Visit

Earn competitive compensation based on your location and visit type:

• All areas except the Bronx and parts of Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint): $70–$75 per visit
• Bronx: Weekday visits – $95 | Weekend visits – $100
• Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint): Weekday visits – $95 | Weekend visits – $100

Annual salary range: USD $85,000.00 - USD $127,500.00
