Your Responsibilities 📝

• Practice independently within the community as a part of an interdisciplinary care team
• Conduct comprehensive patient assessments to determine needs and develop tailored treatment plans
• Deliver personalized rehabilitation therapy in patients' homes or care facilities
• Continuously evaluate and adjust care plans based on evolving patient needs

Qualifications & Compensation 💼

• Minimum of one year of physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern
• Valid license to practice as a Physical Therapist in New York State
• Valid driver's license required outside Manhattan

Pay Range per Visit 💵

If applying for the per diem, per visit position, the standard pay ranges are as follows:

• All areas except the Bronx and certain parts of Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint): $70–$75 per visit
• Bronx: Weekday visits – $95 | Weekend visits – $100
• Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint only): Weekday visits – $95 | Weekend visits – $100

Salary Range 💰

Annual compensation ranges from USD $85,000 to USD $127,500, based on experience and role specifics.
