What You Will Do 🌟

• Practice independently: Serve patients in community settings as part of an interdisciplinary team
• Conduct assessments: Determine patient needs and develop treatment plans with clear outcomes
• Deliver therapy: Provide personalized rehabilitation services in patients’ homes or care facilities
• Adapt care plans: Continually evaluate and respond to evolving patient needs

Qualifications ✔️

• Minimum one year of physical therapy experience or successful completion of probation as a VNS Health Rehab Intern
• Licensure to practice as a Physical Therapist in New York State
• Valid driver's license required for areas outside Manhattan

Pay Range per Visit 💰

If applying for the per diem, per visit position, the standard pay is:

• All areas except: Bronx and parts of Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint): $70–$75 per visit
• Bronx: Weekday visits - $95 | Weekend visits - $100
• Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint): Weekday visits - $95 | Weekend visits - $100

Salary Range 💵

Annual compensation ranges from USD $85,000 to USD $127,500.
