What You Will Do

• Practice independently in the community as part of an interdisciplinary care team 🤝
• Conduct comprehensive patient assessments to determine needs and develop treatment plans 📝
• Deliver personalized rehabilitation therapy in patients’ homes or care facilities 🏠
• Continuously evaluate patient progress and adjust care plans accordingly 🔄

Qualifications

• Minimum of one year of physical therapy experience or successful completion of probation as a VNS Health Rehab Intern 🎓
• Licensure to practice as a Physical Therapist in New York State 📜
• Valid driver’s license required for all areas outside Manhattan 🚗

Pay Range Per Visit

If applying for the per diem, per visit position, the standard pay rate is as follows:

• All areas except the Bronx and parts of Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint): $70–$75 per visit 💵
• Bronx: Weekday visits – $95 | Weekend visits – $100 🏙️
• Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint only): Weekday visits – $95 | Weekend visits – $100 🏙️

Salary Range

Annual salary ranges from USD $85,000.00 to USD $127,500.00.
