What You Will Do 🏡

• Practice independently within the community as part of an interdisciplinary team.
• Conduct comprehensive patient assessments to identify needs and develop effective treatment plans.
• Deliver personalized rehabilitation therapy in patients' homes or care facilities.
• Continuously evaluate patient progress and adjust care plans accordingly.

Qualifications ✅

• Minimum of one-year physical therapy experience or successful completion of probation as a VNS Health Rehab Intern
• Licensure as a Physical Therapist in New York State
• Valid driver’s license required outside Manhattan

Pay Range per Visit 💰

Standard rates apply for per diem, per visit roles:

• All areas except: Bronx and Downtown Brooklyn, Williamsburg, Greenpoint – $70–$75 per visit
• Bronx: Weekday visits – $95 | Weekend visits – $100
• Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint): Weekday visits – $95 | Weekend visits – $100

Annual Salary Range: USD $85,000 – USD $127,500 / year
