What You Will Do 📝

• Practice independently within the community as part of an interdisciplinary care team
• Conduct comprehensive patient assessments to identify needs and develop treatment plans
• Deliver personalized rehabilitation therapies in patients' homes or care facilities
• Continuously evaluate patient progress and adjust care plans accordingly

Qualifications ✔️

• Minimum one-year physical therapy experience or successful completion of a probationary period as a VNS Health Rehab Intern
• Licensure to practice as a Physical Therapist in New York State
• Valid driver’s license required for areas outside Manhattan

Pay Range Per Visit 💰

Note: The following rates apply for per diem, per visit roles:

• Standard Rates (All areas except Bronx and parts of Brooklyn): $70–$75 per visit
• Bronx: Weekday visits — $95 | Weekend visits — $100
• Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint): Weekday visits — $95 | Weekend visits — $100

Salary Range 💵

Annual salary ranging from USD $85,000 to USD $127,500.
