Your Responsibilities

• Practice independently in the community as part of a multidisciplinary care team
• Conduct comprehensive patient assessments to determine needs, develop treatment plans, and set outcomes
• Deliver personalized therapy in patients' homes or care facilities
• Respond to evolving patient needs by adjusting plans of care as necessary

Qualifications

• Minimum of one-year physical therapy experience or successful completion of a probationary period as a VNS Health Rehab Intern
• Current licensure as a Physical Therapist in New York State
• Valid driver's license required for areas outside Manhattan

Pay Range per Visit

If applying for the per diem, per visit position, the standard pay rates are as follows:

• All areas except: Bronx and certain Brooklyn neighborhoods (Downtown Brooklyn, Williamsburg, Greenpoint):
• Weekday visits: $70–$75 per visit
• Bronx: Weekday – $95 | Weekend – $100
• Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint): Weekday – $95 | Weekend – $100

Salary Range

Annual compensation ranges from USD $85,000 to USD $127,500.
