What You Will Do 🚶‍♂️🚶‍♀️

• Practice autonomously within the community as part of an interdisciplinary care team
• Conduct comprehensive patient assessments to determine needs and develop personalized treatment plans
• Deliver individualized rehabilitation therapy in patients' homes or care facilities
• Continuously evaluate patient progress and adjust plans of care as needed

Qualifications ✔️

• Minimum one year of physical therapy experience or successful completion of probation as a VNS Health Rehab Intern
• Licensure as a Physical Therapist in New York State
• Valid driver’s license required for regions outside Manhattan

Pay Range per Visit 💰

If applying for the per diem, per visit position, the standard pay range is:

• Locations outside the Bronx and parts of Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint): $70-$75 per visit
• Bronx - Weekday visits: $95 | Weekend visits: $100
• Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint only): Weekday visits - $95 | Weekend visits - $100

Salary Range 💵

Annual base compensation ranges from USD $85,000 to USD $127,500.
