🤝 Role Description
The Patient Engagement Specialist I is a vital member of the Patient Engagement and Population Health Team. This role involves reaching out to patients via phone and in-person home visits to promote engagement within our care model. The PES I supports patients who haven't yet visited the clinic or participated in our programs, helps them access Oak Street services, and may assist in identifying more suitable primary-care settings.
📝 Responsibilities
- Execute community patient outreach through home visits and phone support (~50% of the time), ensuring reliable transportation.
- Handle escalations and urgent needs by prioritizing, triaging, and managing multiple responsibilities simultaneously.
- Accurately document patient interactions in electronic medical records, strictly adhering to HIPAA standards to maintain confidentiality.
- Facilitate clear communication about complex topics between patients, families, caregivers, medical providers, and community organizations.
- Collaborate with patients and support persons to set goals, overcome barriers, and maintain transparency about expectations.
- Build and cultivate relationships with patients and caregivers independently.
- Partner with Community Health Workers, Social Workers, and clinicians to support patients with complex needs.
- Provide education to patients regarding Oak Street’s comprehensive wrap-around services.
- Aim to achieve at least 20 resolutions per cycle.
🎯 Required Qualifications
- Proven experience in engagement, sales, or marketing within a healthcare-related context.
- Excellent verbal and written communication skills.
- Advanced technological proficiency, including Excel and other EHR/EMR platforms.
- Strong commitment to serving the community and building meaningful professional relationships.
- Ability to work independently in a fast-paced, demanding environment.
- Solid understanding of Medicare and Medicare Advantage programs.
- Dependable transportation and willingness to travel within assigned territories.
- Fluency in Polish (spoken and written) is advantageous.
- Authorization to work in the United States.
