Position: Patient Engagement Specialist (Fluent in Polish)

Role Overview
The Patient Engagement Specialist I will be a vital member of the Patient Engagement and Population Health Team. Your primary responsibilities include outreach efforts—both telephonic and in-home visits—to foster engagement with our care model. You'll support patients who haven't yet visited the clinic or participated in our care programs, helping them access the care they need. Additionally, you may assist patients in finding primary care settings that better suit their needs.
Responsibilities 📝
- Execute community outreach: Conduct home visits and phone support to establish patient connections. (Approx. 50% of your time will be dedicated to community engagement, requiring access to reliable transportation.)
- Handle escalations and urgent needs: Prioritize, triage, and manage multiple responsibilities efficiently.
- Document interactions: Record patient interactions accurately in electronic medical records, ensuring HIPAA compliance and confidentiality.
- Facilitate communication: Act as a connector among patients, family members, caregivers, healthcare providers, and community organizations.
- Collaborate with patients and support persons: Work together to set goals, resolve barriers, and clarify expectations.
- Build relationships: Develop trust and rapport independently with patients and caregivers.
- Partner with care team members: Coordinate with Community Health Workers, Social Workers, and clinicians to address complex patient needs.
- Patient education: Inform patients about Oak Street’s comprehensive wrap-around services.
- Achieve resolution targets: Complete a minimum of 20 patient resolutions per cycle.
What We're Looking For: Requirements & Qualities ⭐
Required Qualifications:
- Proven success in engagement, sales, or marketing within healthcare.
- Strong verbal and written communication skills.
- Advanced technological skills, including proficiency in Excel and EHR/EMR platforms.
- Passion for serving communities and building meaningful relationships.
- Ability to work independently in fast-paced environments.
- Comprehensive knowledge of Medicare and Medicare Advantage programs.
- Dependable transportation and willingness to travel within assigned territories.
- Fluency in Polish (spoken and written): Advantageous.
- Authorization to work in the United States.

Position Details
- Estimated Weekly Hours: 40 hours
- Type: Full-time
- Pay Range: $18.50 – $38.82/hour
The exact salary depends on experience, education, location, and other factors. This position may be eligible for bonuses, commissions, or incentives.
