Position Overview
We are seeking a dedicated Patient Engagement Specialist (PES) to be a key contact point for patient enrollment, outreach, and re-engagement in care. Your role will focus on improving patient access, coordinating referrals, and strengthening relationships through consistent communication and follow-up.
The ideal candidate is bilingual, tech-savvy, comfortable managing high-volume phone interactions, and adept at navigating multiple systems.
Key Responsibilities
- 🔑 Serve as the primary contact for new patient enrollment and re-engagement in care
- 📞 Handle inbound referrals from patients and healthcare organizations via phone and web; provide service information and schedule intakes
- 🔄 Conduct outbound calls to promote services, schedule appointments, and close care gaps
- 🤝 Coordinate internal and external referrals to connect patients with appropriate services
- 🎉 Participate in outreach efforts and health fairs to support community engagement
- 💬 Deliver high-quality customer service, managing complex or sensitive conversations professionally
- 💻 Utilize phone, scheduling, and CRM systems to document activities, manage data, and monitor engagement outcomes
- 📚 Follow communication guides to educate and engage patients effectively
- 📈 Meet outreach and call volume targets, including re-engaging patients who have fallen out of care
- ⏱️ Ensure prompt follow-up on inquiries and voicemails
- 📝 Maintain precise documentation of patient interactions in approved systems
- 🤝 Communicate professionally with internal teams and external partners, representing the organization positively
Requirements
- 🎓 High School Diploma or GED required; college education preferred
- 🈷️ Bilingual proficiency required; Asian language strongly preferred
- 🛎️ Minimum 1 year of experience in customer service, call centers, or sales
- ✍️ Excellent written, verbal, and interpersonal communication skills
- 📱 Ability to manage phone conversations while navigating multiple systems simultaneously
