Job Description
I. Position Summary ๐ฉบ
The primary role of the Patient Care Associate (PCA) is to protect, sustain, and nurture the patients living at the Center. They assist with activities of daily living, care for the patient's environment, and address other needs as required. The goal is to promote a high quality of life for patients through compassionate support. Working as part of a team, the PCA strives to create a home-like atmosphere while maintaining excellence across all areas of care. ๐ก

II. Position Responsibilities ๐
- Recognize and respond to the needs of patients and their families.
- Identify and communicate changes in the patient's condition to the nursing team.
- Adhere to Safe Patient Handling practices, maintaining a no manual lift environment.
- Participate in Quality Improvement programs.
- Assist patients with personal care as outlined in care plans.
- Accurately document care delivered in the electronic medical record.
- Report accidents, incidents, and any observed or suspected patient abuse immediately.
- Observe and report on skin integrity.
- Engage in daily report team and care planning meetings as needed.
- Obtain vital signs and perform procedures such as temperature, pulse, respirations, blood pressure, weight, pulse oximetry, and collection of urine and stool specimens.
- Follow infection control standards diligently.
- Participate in ongoing training and educational sessions.
- Maintain patient confidentiality and privacy in accordance with HIPAA regulations.
- Communicate effectively with peers, team members, patients, and families.
- Facilitate life enrichment activities to promote patient engagement.
- Ensure cleanliness of patient rooms and common areas.
- Assist with meal preparation, serving, and cleanup of meals and snacks.
- Participate in committee meetings and collaborate with team members.
- Perform other duties as assigned and trained.
III. Qualifications ๐
- Long-term chronic care PCA certification or Certified Nursing Assistant (CNA) certification preferred upon hire; current enrollment in nursing school with clinical experience is acceptable.
- RSU PCA: CNA certification required in good standing upon hire and ongoing.
- Fluent in English, both verbal and written.
- Proficient in computer skills.
- Flexible with the ability to work independently and as part of a team.
- Complete all mandatory Hebrew SeniorLife (HSL) / HRC educational requirements and programs.
