Job Description
The primary responsibility of the Patient Care Associate (PCA) is to protect, sustain, and nurture the patients residing at our Center. They assist with activities of daily living, maintain the patient's environment, and address various other needs. The overarching goal is to promote a high quality of life for each patient. Working as a key member of a dedicated team, the PCA strives to create a home-like setting while upholding excellence in all aspects of care.

They actively participate in daily activities, work, and social interactions within the floors/households, aiming to turn every patient encounter into a positive and meaningful experience, all while ensuring safe and efficient care.

Position Responsibilities 📝
- Recognize and respond appropriately to patient and family needs
- Communicate changes in patient conditions to nursing staff
- Adhere to Safe Patient Handling practices, maintaining a no manual lift environment
- Participate in Quality Improvement initiatives
- Assist with all aspects of personal care, following care plans
- Document care provided in the electronic medical records accurately
- Report accidents, incidents, or suspected abuse immediately
- Monitor and report skin integrity concerns
- Engage in daily reports and care planning meetings as needed
- Obtain vital signs and perform procedures such as temperature, pulse, respirations, blood pressure, weight, pulse oximetry, and collect urine/stool samples as directed
- Follow strict infection control protocols
- Participate in ongoing training and educational programs
- Maintain patient confidentiality in accordance with HIPAA regulations
- Communicate effectively with team members, patients, and families
- Facilitate activities that promote patient engagement and well-being
- Ensure cleanliness of patient rooms and common areas
- Prepare, serve, and clean up after meals and snacks
- Participate in committee meetings and other assigned duties
- Perform other duties as assigned and trained
Qualifications 📚
- Certified Nursing Assistant (CNA): Preferred in good standing at hire or current enrollment in nursing school with clinical experience
- RSU PCA Certification: Required and must be maintained in good standing
- Language Skills: Fluent in English, both verbal and written
- Computer Literacy: Ability to use relevant software and electronic health records
- Flexibility: Capable of working independently and within a team
- Educational Requirements: Must complete mandatory Hebrew SeniorLife/HRC training programs
