Job Description

The primary responsibility of the Patient Care Associate is to protect, sustain, and nurture the patients residing at the Center. They assist with activities of daily living, maintain the patient’s environment, and address other needs as they arise. The overarching goal is to promote a high quality of life for our patients while working as part of a cohesive team dedicated to a homelike setting and excellence in all aspects of care.

📝 III. Position Responsibilities

  • Recognize and respond to the needs of patients and families.
  • Communicate any changes in patient conditions to the nursing team promptly.
  • Adhere to Safe Patient Handling practices, maintaining a no manual lift environment.
  • Participate in Quality Improvement initiatives.
  • Assist with all aspects of personal care as outlined in care plans.
  • Document care activities accurately in the electronic medical record.
  • Report accidents, incidents, or suspected abuse immediately.
  • Observe and report skin integrity issues.
  • Engage in daily report meetings and care planning sessions.
  • Proficiently obtain vital signs and perform procedures such as temperature, pulse, respirations, blood pressure, weight, pulse oximetry, and specimen collection.
  • Follow strict infection control practices.
  • Attend and contribute to ongoing training and educational programs.
  • Maintain patient confidentiality in accordance with HIPAA regulations.
  • Communicate effectively with team members, patients, and families.
  • Facilitate activities that encourage patient engagement and enrichment.
  • Maintain cleanliness of patient rooms and communal areas.
  • Assist with meal preparation, serving, and cleanup.
  • Participate in committee meetings and other duties as assigned.

🎓 IV. Qualifications

  • Long Term Care PCAs and MCU PCAs: CNA certification preferred upon hire or current enrollment in nursing school with completed clinical experience.
  • RSU PCAs: CNA certification required in good standing at hire and ongoing.
  • Language Skills: Fluent in English, both verbal and written.
  • Computer Literacy: Required.
  • Flexibility: Ability to work independently and as part of a team.
  • Completion of mandatory Hebrew SeniorLife/HRC educational programs.
  • Pass the PCA skills test with a score of 70% or higher.
