Job Description
Position Summary
The Patient Care Associate (PCA) plays a vital role in protecting, supporting, and nurturing the residents at our Center. ๐ฉโโ๏ธ๐จโโ๏ธ They assist with daily activities, care of the environment, and meet other individual needs. The core goal is to promote a high quality of life and create a home-like atmosphere. Working collaboratively as part of a dedicated team, the PCA strives to uphold excellence across all facets of care, ensuring patients' dignity and choice are always respected. They actively participate in the residents' activities, work, and social environment, transforming every encounter into a positive and meaningful experience while maintaining safe and efficient care standards. ๐
Position Responsibilities
- ๐ฉบ Recognize and respond promptly to the needs of patients and their families.
- ๐ Communicate any changes in the patient's condition to the nursing team.
- ๐ก๏ธ Adhere to Safe Patient Handling practices to maintain a no manual lift environment.
- ๐ Participate in Quality Improvement initiatives.
- ๐ค Assist patients with all aspects of personal care as outlined in care plans.
- ๐ Document relevant information regarding care provided in the electronic medical record.
- ๐จ Report accidents, incidents, or suspected abuse immediately.
- ๐ฉน Observe and report on skin integrity and other clinical indicators.
- ๐๏ธ Contribute to daily report team and care planning meetings.
- ๐ก๏ธ Obtain vital signs and perform procedures including temperature, pulse, respirations, blood pressure, weight, pulse oximetry, and specimen collection as directed.
- ๐งผ Follow infection control protocols diligently.
- ๐ Attend ongoing training and educational sessions.
- ๐ Maintain confidentiality and privacy of patient information according to HIPAA regulations.
- ๐ฌ Communicate effectively with colleagues, patients, and families.
- ๐ Facilitate life enrichment activities to promote patient engagement.
- ๐งน Ensure cleanliness of patient rooms and shared areas.
- ๐ฝ๏ธ Prepare, serve, and clean up after meals and snacks.
- ๐ค Participate in committee meetings and other assigned duties.
- ๐ ๏ธ Perform other duties as assigned and trained.
Qualifications
- ๐ Certified Nursing Assistant (CNA) certification preferred at hire or current enrollment in nursing school with clinical experience.
- โ RSU PCA: CNA certification required at hire and ongoing.
- ๐ฃ๏ธ Fluent in English, both verbal and written communication skills.
- ๐ป Computer literacy is a must.
- ๐ค Ability to work independently and collaboratively, demonstrating flexibility.
- ๐ Completion of mandatory Hebrew SeniorLife/HRC educational programs.
