This clinical position reports to the Clinical Medical Director and plays a pivotal role in making a meaningful difference for a population with the highest levels of obesity and diabetes in the nation.

Essential Job Duties & Responsibilities

• Provide compassionate, high-quality patient care and implement evidence-based medical practices.
• Work within a multidisciplinary team to strategically enhance health outcomes for assigned patients in a coordinated manner.
• Collaborate closely with physicians and health coaches to set wellness goals, promote activation, and improve patient self-efficacy.
• Support onsite and community-based efforts focused on patient engagement, access, population management, referral tracking, mobile clinics, and clinical quality evaluation.
• Perform other duties as assigned.

Qualifications

• Graduate of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program with national certification.
• Minimum 2-5 years of clinical experience, preferably in primary and urgent care settings.
• Valid DEA number.
• Willing to travel weekly between Delano and Lost Hills locations.
• Proficient with technology, including MS Office, tablets, smartphones, and EMR systems.
• Strong interest in driving population health initiatives.
• Fluency in Spanish (written and verbal) preferred.
• Travel required to Lost Hills Clinic, Delano Clinic, and Mobile Clinic facilities.

Knowledge, Skills & Abilities

• Robust background in primary care.
• Experience managing long-term diabetic patients is desirable.
• Ability to provide quality care across diverse environments, including outlying mobile clinics.
• Strong team player with the ability to work independently.

• Salary Range: $150,000.00 - $175,000.00 with eligibility for a discretionary bonus. Final compensation dependent on skills and experience.
