Role Overview
The primary role of a Nurse Practitioner at Oak Street Health is to deliver effective, equitable, and value-based primary care to Medicare-eligible adults. Your goal is to keep patients happy and healthy while reducing hospital visits. Nurse Practitioners operate independently with opportunities for collaboration with physicians, fostering meaningful and trusting relationships with patients to ensure they feel connected to their care plans.
Practicing in outpatient settings, you'll be part of a collaborative care team including physicians, nurse practitioners or physician assistants, medical assistants, dedicated scribes, registered nurses, and care managers. We also partner with specialist networks and hospitals for specialized and acute care, enabling providers to focus on primary care, care coordination, and population health management.
Core Responsibilities
- Deliver exceptional primary care to adult Medicare patients
- Conduct office visits for routine and urgent health issues
- Administer annual wellness visits and health risk assessments with comprehensive documentation
- Coordinate care with specialists, testing facilities, and external agencies
- Lead population health initiatives (e.g., ensuring eligible women receive biennial breast cancer screening)
- Assist with phone triage and patient outreach
- Educate patients about their health conditions, care plans, and treatments
- Participate in promotional activities for Oak Street Health
- Conduct home visits as necessary
- Perform other duties as assigned
This role reports to the Center Medical Director and collaborates closely with operational leadership.
Required Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification in one of the following specialties:
- Family Nurse Practitioner
- Adult-Gerontology Primary Care Nurse Practitioner
- Adult Nurse Practitioner
- Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- US Work Authorization
Preferred Qualifications
- Experience in primary care areas such as internal medicine, geriatrics, or family medicine
- Knowledge of Hierarchical Condition Categories (HCC), ICD-10 coding, and Health Risk Assessments (HRAs)
- Team-oriented mindset with a passion for cross-functional collaboration
- Interest in innovative, empirically-driven population health models
- Bilingual proficiency in applicable languages

Weekly Hours: 40 hours
Time Type: Full-time
Salary Range: $95,738.00 – $206,206.00
The actual starting salary will depend on experience, education, location, and other relevant factors.
