Nurse Practitioner, Advanced Practice Provider

Role Summary
At Oak Street Health, our Nurse Practitioners (NPs) deliver effective, equitable, and value-based primary care tailored for adults on Medicare. They see patients independently while collaborating with physicians as needed, fostering trust through longitudinal care and meaningful relationships. Our NPs are integral to our outpatient practices, working within a collaborative care team and partnering with a network of specialists and hospitals to focus on primary care, care coordination, and population health. 🤝🩺
Core Responsibilities
- Provision of exceptional primary care
- Conduct office visits for routine and acute issues
- Administer annual wellness visits and health risk assessments with a focus on holistic health and accurate documentation
- Care coordination with healthcare providers, specialists, testing facilities, and agencies
- Lead population health initiatives in collaboration with the Care Team (e.g., ensuring timely breast cancer screenings)
- Assist with phone triage and outreach efforts
- Educate patients about their health conditions, care plans, and treatments
- Participate in promotional activities for Oak Street Health
- Conduct home visits as needed
- Perform other duties as assigned
This role reports to the Center Medical Director and collaborates closely with operational leadership.
Required Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate 🎓
- National certification in at least one of the following specialties:
- Family Nurse Practitioner
- Adult-Gerontology Primary Care Nurse Practitioner
- Adult Nurse Practitioner
- Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license ✅
- Active DEA license 🔑
- US Work Authorization 🇺🇸
Preferred Qualifications
- Experience in primary care settings such as internal medicine, geriatrics, or family medicine
- Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments
- Passion for teamwork and cross-functional collaboration
- Interest in an innovative, empirically-guided population health model
- Bilingual proficiency in relevant languages

Anticipated Weekly Hours: 40 hours
Time Type: Full time
Pay Range: $95,738.00 - $206,206.00 annually
This range reflects the full-time base salary and may vary based on experience, education, location, and other factors.
