Nurse Practitioner, Advanced Practice Provider

Role Overview
The Nurse Practitioner at Oak Street Health delivers effective, equitable, and value-based primary care to Medicare-aged adults. 🩺 They see patients independently, collaborate with physicians, and focus on building trust through longitudinal relationships, ensuring patients stay happy, healthy, and out of the hospital. This role involves working within our outpatient practices, supported by a dedicated care team and a network of specialists and hospitals for comprehensive care.
Core Responsibilities
- Provide exceptional primary care services 🏥
- Conduct office visits for routine and acute health issues
- Administer annual wellness visits and health risk assessments with thorough documentation
- Coordinate care with other providers, specialists, testing facilities, and agencies 🤝
- Lead population health initiatives alongside the care team (e.g., ensuring timely breast cancer screenings)
- Assist with phone triage and patient outreach 📞
- Educate patients about their health conditions, care plans, and treatments
- Participate in Oak Street Health promotional activities
- Conduct home visits as needed 🏡
- Perform other duties as assigned
This position reports to the Center Medical Director and works closely with operational leadership to ensure high-quality patient care.
Required Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or graduate of an NP certificate program 🎓
- National certification in one of the following specialties:
- Family Nurse Practitioner
- Adult-Gerontology Primary Care Nurse Practitioner
- Adult Nurse Practitioner
- Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- US Work Authorization
Preferred Qualifications
- Experience in primary care settings such as internal medicine, geriatrics, or family medicine
- Knowledge of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs)
- Passion for teamwork and cross-functional collaboration 🤝
- Interest in innovative, empirically-guided population health models
- Bilingual proficiency in relevant languages

- Pay Range: $95,738.00 – $206,206.00 per year
