Key Responsibilities

• Perform in-home health assessments and physical examinations 🩺
• Diagnose and manage both acute and chronic health conditions
• Develop and implement individualized patient care plans
• Prescribe medications and treatments as appropriate
• Order and interpret diagnostic tests
• Educate patients and families on health management and disease prevention
• Coordinate care with physicians and other healthcare providers
• Ensure accurate and timely medical documentation

Qualifications & Requirements

• Active Nurse Practitioner license in New York State ✅
• Board Certification as a Nurse Practitioner 🎓
• Master of Science in Nursing (MSN) or higher degree
• Mandarin language proficiency is REQUIRED 🈷️
• Strong clinical assessment and diagnostic abilities
• Ability to work independently in home-care settings
