🎓 Qualifications

• Education: Master of Science in Nursing (MSN) or higher from an accredited Nurse Practitioner program
• Licensure: Active New York State Nurse Practitioner License (Required)
• Certification: Board Certified Nurse Practitioner
• Experience: New graduates are encouraged to apply
• Language: Bilingual in Mandarin
• Technical Skills: Proficiency in patient assessments, diagnosis, treatment planning, documentation, and familiarity with point-of-care testing and portable medical equipment
• Soft Skills: Highly independent, excellent clinical judgment, strong communication, compassionate, and adaptable to various home environments

📝 Key Responsibilities

• Perform comprehensive health assessments and physical examinations in patients' homes
• Diagnose and manage acute and chronic health conditions
• Develop and implement individualized care plans
• Prescribe medications and treatments within the NP scope of practice
• Order and interpret diagnostic tests
• Provide patient and family education on health maintenance and disease management
• Coordinate care with primary care physicians and other healthcare providers
• Maintain accurate and timely documentation of all patient encounters
