Position Details
- Title: CareBridge Advance Practice Provider, Nurse Practitioner (Bilingual)
- Location: Virtual (with occasional in-person training; alternate locations considered within commuting distance)
- Work Schedule: Monday – Friday, 8:00 am to 5:00 pm EST, with rotating on-call 2-3 days/month from 5:00 pm to 8:00 am EST

Key Responsibilities
- Collaborate with physicians, providers, and families to develop complex, personalized care plans aligning with patients’ goals.
- Provide primary and urgent care via telephone and tele-video, supporting Medicaid, dual-eligible members, and other assigned populations.
- Assess patient histories, conduct physical exams, diagnostics, and develop treatment strategies.
- Identify gaps in care and implement interventions to address them.
- Support patients and families, coordinating with interdisciplinary teams to meet physical and psychosocial needs.
- Educate on medication use, side effects, disease progression, nutrition, adherence, and crisis prevention.
- Maintain accurate medical records and documentation following state and company standards.
- Participate in ongoing continuing education and professional development.
- Prescribe medications as permitted under state regulations.
Minimum Qualifications
- MS in Nursing
- Active national Nurse Practitioner certification
- Valid, unrestricted Family or Adult Nurse Practitioner license in New York
- Valid RN license
- Experience with Electronic Medical Records (EMR)
- At least 2 years managing complex care cases
- Bilingual skills in Spanish, Vietnamese, Mandarin, or Russian required
Preferred Skills & Experience
- Active RN Compact license
- Experience with developmental disabilities and chronic illness management
- DEA registration or eligibility
- Bilingual in Spanish (highly preferred)
- Active Medicaid number in New York (highly preferred)
Compensation & Additional Information
The salary range for this role is $110,080 to $198,144. Compensation includes benefits and perks aligned with the company's policies and applicable laws.
