🔹 Position Details

• Title: CareBridge Advance Practice Provider, Nurse Practitioner (Bilingual)
• Location: Remote (within commuting distance of New York)
• Work Schedule: Monday - Friday, 8:00 am to 5:00 pm EST, with rotating on-call 2-3 days/month from 5:00 pm to 8:00 am EST
• Sign-on Bonus: $5,000
• Salary Range: $110,080 to $198,144 (based on experience and location)

🚀 How You Will Make an Impact

• Provide primary and urgent care via telephone and tele-video to home and community-based patients
• Develop and implement detailed care plans for adults with complex and chronic conditions
• Gather patient histories, conduct physical exams, and coordinate diagnostics
• Identify gaps in care and take steps to close them
• Support patients and families in meeting physical and psychosocial needs, collaborating with interdisciplinary teams
• Educate patients on medication use, side effects, chronic disease management, and crisis prevention
• Maintain timely communication with other healthcare providers for coordinated care
• Document patient records in compliance with state regulations and organizational policies
• Participate in ongoing education and training programs
• Prescribe medications within the scope of state licensing

🔹 Qualifications & Requirements

• Master’s degree in Nursing (MSN) 🎓
• Active, unrestricted Family or Adult Nurse Practitioner license in New York
• Current RN license
• Minimum of 2 years managing complex care cases
• Experience with Electronic Medical Records (EMR)
• Bilingual proficiency in Spanish, Vietnamese, Mandarin, or Russian is required

🌟 Preferred Skills & Extras

• Active RN Compact license
• Experience with developmental disabilities and chronic illness management
• Possession of DEA registration or eligibility
• Highly preferred: Bilingual in Spanish
• Preferred: Medicaid number in New York
