CareBridge Advance Practice Provider, Nurse Practitioner (Bilingual)

💰 Sign-on Bonus: $5,000

📍 Location: Remote (Virtual, NY-based candidates preferred)

We are seeking licensed Bilingual Nurse Practitioners in New York who hold an active RN license to join CareBridge Health, a proud member of the Elevance Health family. This role offers full-time remote work with occasional in-person training sessions, providing maximum flexibility and autonomy.

🕒 Work Schedule

• Monday – Friday, 8:00 am – 5:00 pm EST
• Rotating on-call 2-3 days per month, from 5:00 pm to 8:00 am EST

🌐 Role Responsibilities

• Collaborate with physicians, healthcare providers, and patients' families to develop comprehensive care plans aligned with patient goals.
• Provide primary and urgent care via telephone and tele-video to Medicaid and dual-eligible members in the community.
• Develop and implement clinical plans for adults with chronic and complex conditions, including mental health diagnoses.
• Gather medical history, conduct physical exams, and develop treatment strategies based on current conditions and patient goals.
• Identify gaps in care and work to close them efficiently.
• Support patients and families, addressing physical and psychosocial needs with team input.
• Educate on medications, illness progression, nutrition, adherence, and crisis prevention.
• Coordinate care with interdisciplinary teams and external providers, ensuring timely and compliant delivery of services.
• Maintain accurate medical records as per regulations and company policies.
• Engage in continuing education as required by legal and certifying bodies.
• Prescribe medications within state authority.

🔑 Minimum Qualifications

• MS in Nursing
• Active national Nurse Practitioner certification
• Unrestricted Family or Adult NP license in New York
• Current RN license
• Experience with Electronic Medical Records (EMR)
• 2+ years managing complex care cases
• Bilingual skills required in Spanish, Vietnamese, Mandarin, or Russian

✨ Preferred Skills & Experience

• Active RN Compact license
• Experience with developmental disabilities and chronic illness management
• DEA registration or eligibility
• Fluent Spanish language skills strongly preferred
• Active Medicaid number in NY highly preferred

💼 Compensation & Benefits

Salary range for this role: $110,080 – $198,144
