Salary Range: $139,150 - $163,350

Role Overview 🚀
The Nurse Practitioner at Oak Street Health delivers effective, equitable, value-based primary care to Medicare-eligible adults. They independently see patients, collaborate with physicians, and foster meaningful relationships that create trust and improve health outcomes. Our outpatient practices support a collaborative care team including physicians, nurse practitioners, PAs, medical assistants, and dedicated support staff, all working together to focus on primary care, care coordination, and population health.
Core Responsibilities 📝
- Provision of exceptional primary care services
- Conduct routine and acute office visits
- Administer annual wellness visits and health risk assessments with holistic health views and precise documentation
- Coordinate care with other providers, specialists, testing facilities, and agencies
- Lead population health initiatives (e.g., ensuring eligible women receive biennial breast cancer screenings)
- Assist with phone triage and outreach efforts
- Educate patients on their health conditions, care plans, and treatments
- Participate in Oak Street Health promotional activities
- Conduct home visits as needed
- Perform other duties as assigned
This role reports directly to the Center Medical Director and collaborates with operational leadership.
Required Qualifications ✅
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certification
- Certification in at least one specialty:
- Family Nurse Practitioner
- Adult-Gerontology Primary Care Nurse Practitioner
- Adult Nurse Practitioner
- Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- US Work Authorization
Preferred Qualifications 🌟
- Experience in primary care settings such as internal medicine, geriatrics, or family medicine
- Understanding of HCC documentation, ICD-10 coding, and HRAs
- Team-oriented mindset with a passion for cross-functional collaboration
- Interest in innovative, empirically-guided population health models
- Bilingual proficiency in relevant languages

Pay Range:
The typical salary range for this role is $104,442.00 - $224,952.00. Actual offers depend on experience, education, location, and other relevant factors.
