Position Title: Oak Street Health New Graduate Nurse Practitioner Fellowship

Location: Soundview - 1555 Westchester Ave, Bronx, NY 10472

🔍 Fellowship Program Overview

Kickstart your NP career in the evolving landscape of healthcare! In collaboration with the University of Michigan School of Nursing, Oak Street Health’s NP Fellowship in Adult Primary Care offers an intensive 12-month paid program designed to develop confident, competent clinicians for older adults. Set within a revolutionary value-based care model, this program emphasizes quality over quantity, rewarding excellent patient care.

Throughout the fellowship, participants will:

• Build their own patient panel
• Learn from diverse preceptors
• Engage in advanced didactic content tailored to enhance graduate training

Post-fellowship, graduates commit to working at Oak Street Health for at least a year, with many choosing to establish lifelong careers dedicated to community health improvement. The program begins in September and March annually, with two cohorts each year.

Annual Salary for this location: $112,000

🔎 Applicant Requirements

• Graduates or soon-to-be graduates of family, adult, or adult-gerontology primary care NP programs (Dec 2024 - May 2026)
• Willing to reside or relocate to the Bronx location for at least two years
• Not yet employed as an NP
• At least one year of recent adult RN experience (within the last five years)
• Minimum GPA of 3.0 in NP program
• Passing or in-process of passing NP board exams by 6/6/2026
• Active, non-probationary state NP license by 8/3/2026
• Passion for adult primary care and a supportive attitude towards older adults
• Team-oriented mindset and enthusiasm for innovative, population health-focused models
• Spanish language skills are strongly preferred, depending on clinic location

💰 Compensation & Benefits

Pay Range: $104,442 – $224,952 annually

This range reflects the base salary and may vary based on experience, education, location, and other factors.
