Small Joys provides integrated behavioral health services by partnering with primary and specialty care physicians to support patient wellbeing and improve health outcomes. We are currently seeking a dedicated Behavioral Health Integration Manager for a full-time, in-person role supporting our clinic locations.

Responsibilities

• 🤝 Conduct patient intake assessments and assist in developing individualized treatment plans
• 🔄 Provide follow-up sessions and care coordination for patients enrolled in behavioral health programs
• 📝 Document clinical notes using standardized templates and maintain accurate patient records
• 🧠 Collaborate with psychiatrists and primary care physicians regarding care plans and patient progress
• 🔗 Act as a liaison between the behavioral health care team and primary care providers
• 📊 Manage patient registry and track time spent on each patient for billing and reporting
• 👪 Coordinate with patients’ family members when necessary to gather medical and behavioral history
• 🚑 Support care delivery across both clinic locations

Qualifications

• 🎓 Master’s degree in Clinical Psychology, Counseling, or a related field
• 🎓 Master of Social Work (MSW) also considered
• 🩺 Minimum 2 years of relevant behavioral health experience
• 🗣️ Bilingual proficiency in English and Korean required
• ✍️ Strong documentation, coordination, and communication skills
• 🤝 Ability to work collaboratively with multidisciplinary healthcare teams
