Position Summary
The Medical Records Clerk is responsible for organizing and maintaining patients' medical records, including personal details, medical histories, and billing information. This role involves precise data entry, creating efficient filing systems, and ensuring compliance with industry standards. The Clerk collaborates with various departments to facilitate smooth admissions, discharges, and record retrieval, maintaining accuracy and completeness of all documentation.
Key Responsibilities
- Assist in answering and directing calls to the main home care line 📞
- Access, maintain, and audit all medical records ensuring accuracy 🗃️
- Send Plan of Care and Supplemental Orders to Primary Care Physicians for signatures; follow up on outstanding orders to meet regulatory deadlines ⏱️
- Distribute Transfer and Discharge Summaries to Primary Care MDs within regulated time frames 📄
- Scan signed MD orders and attach them to the electronic patient record 🖥️
- Track home health admissions and discharges in Patient PING ⚕️
- Communicate patient hospitalization status to clinical and operational staff 🏥
- Support accounts receivable processes for service reimbursement 💵
- Organize off-site storage with updated logs for seamless record retrieval 📁
- Create Start of Care (SOC) packets for home health admissions 📦
- Maintain the Emergency Preparedness Binder current 🚑
- Track outstanding clinical documentation and follow up with staff as needed ✉️
- Manage Tiger Text submissions, saving and attaching documents to EMR via Tiger Connect 📲
- Participate in staff meetings and ongoing education for personal and professional growth 📚
- Track office supplies and manage mail/packages 📦✉️
- Facilitate discharge tasks within EMR 🔄
- Ensure compliance with federal, state, and local regulations, including HIPAA privacy and security policies 🔒
- Support data entry for admissions and agency messaging (email management) 💻
- Complete special projects as assigned and perform other related duties 📝
Qualifications
- Proficient computer skills 💻
- Experience with electronic documentation systems (Allscripts preferred) 🗂️
- Excellent verbal and written communication abilities 🗣️✍️
- Motivated to learn, adaptable, and flexible to change 🔄
- Strong customer service skills 🤝
- Knowledge of Medicare regulations for certified home care is a plus 🩺
