Position Summary

The Medical Records Clerk is responsible for organizing and maintaining patients' medical records, including personal information, medical history, and billing records. This role involves data entry, creating and managing filing systems, ensuring records are complete and accurate, and collaborating with other departments to facilitate smooth admissions, discharges, and record retrieval.

Key Responsibilities

  • Assist in answering and directing calls on the home care line 📞
  • Access, maintain, and audit all medical records efficiently 🗂️
  • Send Plans of Care and Supplemental Orders to Primary Care Physicians for signatures ✉️
  • Follow up with MD offices to ensure orders are returned within 30 days for compliance ✅
  • Send Transfer and Discharge Summaries to Primary Care MD within regulated timeframes 🕒
  • Scan signed MD orders and attach to electronic patient records 🖥️
  • Track home health admissions and discharges in Patient PING 📊
  • Support hospitalizations by communicating with clinical and operational staff 🏥
  • Assist with accounts receivable tasks for service reimbursement 💰
  • Organize and maintain off-site storage logs for seamless record retrieval 📁
  • Create SOC packets for all home health admissions 📂
  • Maintain the Emergency Preparedness Binder current 🚨
  • Follow up on outstanding clinical documentation from staff 📋
  • Manage Tiger Text submissions of all clinical documents via Tiger Connect 📱
  • Save and attach documents to EMR accurately 🖱️
  • Participate in staff meetings and ongoing training for professional growth 💡
  • Track office supplies and manage incoming mail 📬
  • Facilitate discharge tasks within EMR 📝
  • Create an agency environment compliant with federal, state, and local regulations 🛡️
  • Ensure HIPAA privacy and security standards are met 🔒
  • Support data entry for admissions as needed 📝
  • Manage agency messaging and email communications 📧
  • Complete various special projects as assigned 🎯

Qualifications

  • Proficient computer skills 💻
  • Experience with electronic documentation systems (Allscripts experience a plus) 📄
  • Excellent verbal and written communication skills 🗣️✍️
  • Motivated to learn and adaptable to change 🔄
  • Strong customer service skills 🤝
  • Knowledge of Medicare regulations for certified home care (preferred) 🏥
