Schedule & Location
- Schedule: Monday - Friday, 9am - 5pm
- Location: On-site in Brooklyn, NY 11218
Key Responsibilities
Virtual Visit Facilitation
- Initiate outbound calls to patients at scheduled times to prepare and connect them with their provider via video or telephone.
- Ensure patients are ready for the visit, confirming availability, technology readiness, and required documentation.
- Coordinate real-time handoff between patient and provider to support on-time start of visits.
Appointment Reminders & Outreach
- Make appointment reminder calls, same-day confirmations, and follow-up outreach to minimize no-shows.
- Communicate schedule changes, delays, or cancellations to patients and providers as needed.
- Document all outreach efforts and patient interactions according to organizational standards.
Provider Schedule Support
- Monitor provider schedules and proactively address gaps, late arrivals, or connectivity issues.
- Serve as the first point of contact for visit-related operational issues during clinic hours.
Documentation & Systems
- Accurately record visit statuses, outreach attempts, and outcomes in electronic health records (EHR) and other systems.
- Keep appointment notes and visit readiness flags current, adhering to HIPAA and confidentiality policies.
Coordination & Communication
- Collaborate with providers, medical assistants, nurses, and front-office staff to ensure smooth visit flow.
- Provide professional, clear communication to patients regarding visit expectations and next steps.
- Identify recurring barriers or scheduling issues to support continuous process improvement.
Required Qualifications
- High School Diploma or GED
- Fluency in Cantonese (bilingual ability)
- 1-2 years of telehealth experience
- Proficiency with EMR systems; experience with AdvancedMD is a plus
- Strong knowledge of patient care procedures and medical terminology
- Excellent interpersonal skills for effective patient and team interactions
- Strong organizational abilities to manage patient records and schedules
