Key Responsibilities
Virtual Visit Facilitation
- Initiate outbound calls to patients at scheduled appointment times to prepare and connect them to their provider (video or telephone visits).
- Confirm patient availability, technology readiness, and required documentation to ensure they are ready for the visit.
- Coordinate real-time handoff between patients and providers to support timely visit start.
Appointment Reminders & Outreach
- Conduct reminder calls, same-day confirmations, and follow-up outreach to reduce no-shows.
- Communicate schedule changes, delays, or cancellations to patients and providers as needed.
- Document all outreach attempts and patient interactions following organizational standards.
Provider Schedule Support
- Monitor provider schedules throughout the day and proactively address gaps, late arrivals, or connectivity issues.
- Serve as the first point of contact for visit-related operational issues during clinic hours.
Documentation & Systems
- Accurately document visit status, outreach, and outcomes in designated systems (e.g., EHR, scheduling platform).
- Maintain current appointment notes and flags related to visit readiness or barriers.
- Follow HIPAA and organizational confidentiality policies at all times.
Coordination & Communication
- Work closely with providers, medical assistants, nurses, and front office staff to ensure smooth visit flow.
- Provide clear, professional communication to patients regarding visit expectations and next steps.
- Identify recurring visit barriers or scheduling issues to support process improvements.
Required Qualifications
- High School Diploma or GED
- Bilingual fluency in Cantonese
- 1-2 years of telehealth experience
- Proficiency in EMR systems; experience in Advanced MD is a plus
- Strong understanding of patient care procedures and medical terminology
- Excellent interpersonal skills for effective interaction with patients and healthcare professionals
- Strong organizational skills to manage patient records and scheduling
