Schedule & Location

• Schedule: Monday - Friday, 9am - 5pm
• Location: On-site in Brooklyn, NY 11218

Key Responsibilities

🩺 Virtual Visit Facilitation

• Initiate outbound calls to patients at scheduled appointment times to prepare and connect them with their provider (video or telephone visits).
• Ensure patients are ready for their visits, including confirming availability, technology readiness, and required documentation.
• Coordinate real-time handoffs between patients and providers to support on-time visit starts.

🔔 Appointment Reminders & Outreach

• Conduct appointment reminder calls, same-day confirmations, and follow-up outreach to reduce no-shows.
• Communicate schedule updates, delays, or cancellations to patients and providers as needed.
• Document all outreach attempts and patient interactions in organizational systems.

📅 Provider Schedule Support

• Monitor provider schedules throughout the day and proactively address gaps, late arrivals, or connectivity issues.
• Serve as the primary contact for managing visit-related operational issues during clinic hours.

📝 Documentation & Systems

• Accurately document visit status, outreach efforts, and outcomes in designated systems (e.g., EHR, scheduling platform).
• Keep appointment notes and flags up-to-date, noting visit readiness or barriers.
• Maintain strict confidentiality following HIPAA and organizational policies.

🤝 Coordination & Communication

• Collaborate with providers, medical assistants, nurses, and front office staff to ensure seamless visit flow.
• Provide clear, professional guidance to patients regarding visit expectations and next steps.
• Identify recurring barriers or scheduling issues to support process improvements.

Required Qualifications

• High School Diploma or GED
• Bilingual Fluency in Russian or Chinese
• 1-2 years of telehealth experience
• Proficiency with EMR systems (experience with AdvancedMD is a plus)
• Strong understanding of patient care procedures and medical terminology
• Excellent interpersonal skills for effective interaction with patients and healthcare team members
• Strong organizational skills for managing patient records and scheduling
