Position Summary

The Medical Assistant plays a vital role in supporting a team of nurses and nurse practitioners to deliver exceptional patient care. Key responsibilities include remote vital signs monitoring, accurate documentation, patient inquiries, medical record management, and ensuring compliance with RPM and healthcare regulations.

Key Responsibilities

  • Collaborate with Nurse Practitioners, Care Navigators, and administrative staff to monitor patient health metrics, ensure accurate data entry, and coordinate follow-up care. Act as a liaison between patients and care teams for RPM and CCM services.
  • Address alerts on dashboards and communicate promptly with patients as needed.
  • Demonstrate excellent phone communication skills, conveying empathy and professionalism.
  • Remind patients to take necessary measurements and accurately record results in patient charts.
  • Escalate patient calls to nurses or Nurse Practitioners when further assessment is necessary.
  • Maintain and update patient medical histories, vital signs, and test results within the Electronic Medical Record (EMR) system.
  • Manage inbound and outbound calls, address inquiries, monitor vital signs via RPM dashboards, and document interactions in the EMR. Follow up to promote adherence to care plans.
  • Perform data entry in Salesforce, including closing opportunities and tracking progress.
  • Conduct outbound calls to provide instructions, collect information, and address patient concerns.
  • Utilize EMR systems like Advanced MD and CCIQ for data management.
  • Verify insurance coverage and secure prior authorizations as needed.
  • Schedule virtual appointments with Nurse Practitioners, coordinating with patients and the healthcare team.
  • Ensure compliance with healthcare regulations such as HIPAA and uphold organizational standards.

Required Qualifications

  • High School Diploma or GED
  • Bilingual Fluency in Cantonese is required
  • 1-2 years of relevant healthcare experience
  • Proficiency with EMR systems; experience with AdvancedMD is a plus
  • Good understanding of patient care procedures and medical terminology
  • Excellent interpersonal skills for effective communication with patients and healthcare professionals
  • Strong organizational skills for managing records and scheduling appointments
  • Exceptional customer service skills to provide empathetic patient interactions
